We need certain documentation for each person traveling on a mission’s trip to our facility. If you are traveling with your local church on a team to our facility, ask your team leader which documents that they require from each Team Member, but they will probably ask for something similar to what is listed below.
For those individuals traveling alone or on long term trips lasting longer than a short term trip, we need the following documentation ahead of time:
- An Internship Application or Missions Application
- Record of Vaccinations
- Copy of your Passport
- Copy of your travel insurance
- A signed Ministry Agreement and Release of Liability Form
- All minors traveling without their parent or guardian must have parental or guardian permission to travel to our facilities. Please contact us for more information on how to verify parent/guardian permission, and additional documentation necessary.
